Terms and Conditions of Use for Website and Patient Portal (RXNT)
The Catoosa Behavioral Health and Wellness Clinic, LLC ("Catoosa Wellness") website (https://catoosawellness.com) and the integrated RXNT patient portal service (collectively, the "Service") are provided solely for the convenience of patients of Catoosa Behavioral Health and Wellness Clinic, LLC, as identified in its Notice of Privacy Practices.
Your access to and use of the Service, whether through the website (the "Site") or the RXNT mobile application (the "App"), is subject to and governed by these Terms and Conditions of Use ("Terms and Conditions" or "Agreement") and applicable laws, rules, and regulations.
Throughout this Agreement, Catoosa Behavioral Health and Wellness Clinic, LLC is referred to as "we," "us," or the "Clinic." Users of the Service will be referred to as "you" or "Users."
1. Emergency Disclaimer
NEVER USE THIS SERVICE FOR EMERGENCY OR OTHER URGENT MEDICAL ISSUES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY OR NEED URGENT MEDICAL CARE, IMMEDIATELY CALL 911 (IF YOU ARE IN THE UNITED STATES), GO TO YOUR LOCAL EMERGENCY ROOM, AND/OR CALL YOUR PHYSICIAN.
All health-related information provided through this Service is intended to be general in nature, provided solely for patient convenience, and is not a substitute for a visit to a licensed healthcare provider. Please see your physician for all medical advice and care.
2. Privacy and Device Access
If you use the RXNT App, please be aware that the App will interact with your protected health information (PHI) to provide features such as image uploads or mobile appointment check-in. The first time you use such a feature, the App will request permission to access certain data on your device (such as location data or camera access).

Location Data: Used solely to facilitate the check-in process when you arrive for an appointment or to find nearby providers. The App does not store your location data.

Images: Images you upload to the App will be retained in your official medical records.

Push Notifications: You may opt-in for automatic reminders (appointments, test results). These notifications may be displayed on a locked screen and, similar to text messages, are unsecure communications that could be viewed by third parties. Your decision to opt-in is voluntary and can be revoked at any time in your device settings.
3. Purpose of the Service
The Service currently provides a convenient way for you to:

Have web-based or mobile-based, read-only access to portions of your health information.

Use secure messaging to communicate with Clinic healthcare providers.

Receive select lab results from your physician.

Review, make, or cancel appointments.

Request prescription refills.

View and pay Clinic bills online.
Please Note: The patient portal is not your complete medical record, nor is it your official medical record. To request a complete copy of your medical record, please contact the Clinic office directly.
4. Personal and Proxy Accounts

Personal Accounts: Only competent adults or legally emancipated minors may request a personal account.

Minor (12-17) Accounts: Parents or legal guardians may request a "Minor Account" for unemancipated minors aged 12-17. Access to certain information for minors in this age range may be limited by state and federal law to protect the minor's privacy regarding specific sensitive treatments.

Proxy Access: A competent adult may be granted access to another patient’s account (e.g., an elderly parent or a child under 12) if they are determined to be legally authorized to access that patient's health information.
5. Electronic Messaging and Email Privacy
Secure messaging should never be used for urgent needs. We will respond to messages as timely as possible, but it is your responsibility to monitor your account for responses.
All messages sent through the Service become part of your permanent medical record. Because messages are routed to the appropriate clinical staff to ensure a timely response, do not include information you wish to discuss only with your specific physician.
6. Remote Patient Monitoring (RPM)
You may choose to permit the Clinic to connect the Service to certain electronic devices (e.g., heart rate monitors, blood pressure cuffs, or fitness trackers). This data may be stored in your electronic health record. Participation in any remote monitoring program is entirely voluntary and can be withdrawn at any time without affecting your right to future care.
7. Online Payments
We use a third-party processor to handle payments. By using this feature, you agree to comply with the terms and conditions of that third-party processor.
8. Data Security
The Service must be accessed with a Secure Sockets Layer (SSL) compatible web browser (Edge, Chrome, Safari, or Firefox). To reduce the risk of unauthorized access, your account will be disabled if your password is entered incorrectly three (3) times within a 24-hour period.
Your Responsibility: You are solely responsible for maintaining the confidentiality of your login ID and password. We recommend changing your password regularly. We are not responsible for security failures resulting from your own acts or omissions (e.g., failing to log out of a public computer or sharing your password).
9. Intellectual Property
All content on the Site and App is the property of Catoosa Behavioral Health and Wellness Clinic, LLC or its licensors (such as RXNT). You are granted a limited, non-exclusive right to use the Service for personal, non-commercial purposes only.
10. Termination
The Clinic reserves the right to terminate this Agreement or modify/discontinue the Service at any time, without notice or cause. This may occur if you are no longer a patient, in the event of legal issues, or if we determine a violation of these Terms has occurred.
11. Limitation of Liability
To the maximum extent permitted by law, the Clinic and its affiliates shall not be liable for any indirect, special, incidental, or consequential damages arising from your use of the Service. Your sole and exclusive remedy for dissatisfaction with the Service is to stop using it.
12. Choice of Law and Venue
This Agreement is governed by the laws of the State of Georgia. You agree that any legal action arising from this Agreement shall be brought exclusively in the state or federal courts sitting in Catoosa County, Georgia.

By signing up for or using the Catoosa Wellness Website or RXNT Portal, you acknowledge that you have read, understood, and voluntarily agree to these Terms and Conditions and our Notice of Privacy Practices.
